Hammerlee Dental Care is a welcoming, friendly, family-oriented dental practice in Erie, PA. We are proud to provide a full range of cosmetic and family dental services. Our goal is provide each and every patient with the best dental care. We focus on helping patients feel welcome and comfortable in our office and take pride in forming connections with each individual. Our staff is dedicated to providing the highest level of personalized, gentle and efficient dental care.

What to Bring to your First Appointment

Hammerlee Dental Care is currently accepting new patients. Please call our office at 814-868-3647 to schedule an appointment. Once you make an appointment at our office, that time will be reserved for you. If, for some reason, you are unable to keep your scheduled appointment time, we ask for a minimum of 24 hours notice for changes in appointments. We will work with you to reschedule.

To provide you with prompt dental treatment, please be sure to bring the following when you visit our practice for your initial visit:

  • Insurance Information/Card
  • Photo Identification
  • Any medical records pertaining to your visit

When you make your first appointment, we will give you instructions on how to fill out all of the required forms through our online, secure portal. You must have an appointment scheduled in order to access these forms. We request you fill out these forms in advance of your appointment to save time during your visit. You can fill these forms out at your convenience and at home prior to your visit. The required forms can be found below.

Payment Information

We do not want finances to be an obstacle in obtaining your dental health needs. We have developed a number of options to help with the payment of your treatment.

You may pay for your visits and any treatments by cash, check, money order, credit cards, payment plans and most insurance plans.

For our patients with dental insurance, we gladly provide you with the benefit of participating with many plans. We are happy to assist you with utilizing your dental benefits and are available to answer any questions you may have.

We are proud to offer our patients opportunities to finance their treatment by making monthly payments. We have arrangements for financing with Citibank and CareCredit for your convenience. Zero interest loans, for six or twelve months, are often available. Feel free to contact us for financing details.
